Authorization for Repairs and Services
I, the undersigned, as the Owner or Authorized Representative (hereinafter referred to as “Owner”) of the property located at the Jobsite Address where work is to be performed, hereby authorize Able Roofing and Restoration, Inc. (“Able Roofing”) to carry out the specified repairs or services. I grant permission for Able Roofing to utilize labor, materials, and equipment as it deems appropriate to complete the work.
Scope of Repairs
The repairs undertaken by Able Roofing are based solely on a visual inspection of the area identified as problematic. No destructive testing or water testing will be conducted prior to the commencement of work unless explicitly requested by the customer in writing and paid for accordingly. Unless otherwise agreed in writing, Able Roofing will limit its assessment to a visual inspection and cannot guarantee that no additional issues will be discovered during the repair process.
Authorization and Acknowledgment
By signing this document, I authorize Able Roofing to perform the work described under the terms outlined above. I confirm that I have the authority to approve repairs at the property specified and agree to pay for the services in accordance with the stated Terms. If the estimated cost of the work is expected to exceed the standard or after-business/weekend limits, a written estimate will be provided, and this Repair Work Authorization will be replaced by the new agreement.
Electronic Signature Consent
I understand and acknowledge that by accepting this agreement, I am providing my electronic signature, granting Able Roofing permission to proceed with the work. This consent complies with all applicable Illinois laws, specifically including the Uniform Electronic Transactions Act (UETA), as adopted by the State of Illinois under 5 ILCS 175/1-101 et seq., which establishes the legality of electronic signatures and records.
